Program Designed to Reduce or Avoid Unnecessary Hospitalizations, Readmissions and Emergency Room Visits
Soleo Health, an innovative leader and national provider of complex specialty pharmacy and infusion services, announced today the capabilities of its new Soleo Direct™ program. Soleo Direct is a care path alternatives program designed to reduce or avoid unnecessary hospitalizations, readmissions and emergency department visits.
The Soleo Direct program assists hospitals, health systems, physicians, health plans and payors in identifying appropriate patients eligible for receiving treatment in alternate sites of care through multidisciplinary collaboration, as they are discharged. Soleo Direct utilizes proprietary algorithms to determine targeted diagnosis-related groups (DRGs) and establishes care paths for specific diagnoses that can safely be treated with intravenous or injectable medications in the home or alternate site setting.
Soleo Direct specifically customizes care paths using these clinical algorithms for hospital systems to help facilitate the discharge planning process while also collecting reportable data metrics to provide deeper intelligence. It has the ability to track readmissions in both outpatient and transitions of care settings, resulting in evidence-based data that helps better manage patient care plans utilizing Soleo Health’s proprietary clinical outcomes program, SoleMetrics®.
In a recent study of cellulitis patients conducted over a one-year period, Soleo Health used Soleo Direct to deliver clinical interventions, which enabled the transition of patients to lower cost of care sites, directly bypassing emergency departments and hospitalizations. The study resulted in cost savings of nearly $500,000 as revealed by data provided through SoleMetrics.
"Through Soleo Direct, Soleo Health clinicians work directly with physicians and hospital staff to identify those patients at risk for both admission and/or readmission, who would be capable of receiving clinical treatment in their homes or at alternate sites. Soleo Health provides patient status updates via Soleo Connect®, an online portal offered to physicians and patients. This dedicated, personalized approach supports physicians’ care and instills their confidence in the fact that their patients are being closely monitored and their medical status recorded. This assessment data leverages evidence-based standards of care, which proves extremely valuable to all constituents involved in the plans of care," said Barbara Prosser, vice president, health outcomes and research.
"By working collaboratively with hospitals, health systems, payors and physicians, Soleo Direct is emphasizing medication stewardship, both during and after a hospitalization or emergency room visit, thereby positively impacting the patient’s quality of life. Patients are able to return to school and work and resume daily activities of life," Prosser added.
"With our new cutting-edge, data-driven program, we can cost-effectively manage and care for patients in a comfortable yet controlled environment, such as their home or ambulatory infusion site, removing them from potentially higher-cost settings where they are also susceptible to infection. Soleo Direct helps avoid unnecessary hospitalizations and emergency department visits, while facilitating ease of discharge by decreasing length of stay. This reaps significant costs savings for the healthcare system. As we further expand the Soleo Direct program to hospital systems and payors nationally, even greater savings can be realized through our ability to monitor, measure and mitigate risk by leveraging the patient outcomes metrics it reveals," said Drew Walk, Soleo Health’s chief executive officer.
